When Theo gave Alicia's husband the choice to save his wife or his own life, he wanted Alicia to reject her husband as an adulterer. He leaves them before she kills her husband, but with Alicia's silence he doesn't know why she killed him. Her 6 years of silence makes Theo feel safe and his psychiatric ego makes him think he can help her. The only plot twist

I think I can answer without spoiling the book. Just like in Euripides' play, why won't Alcestis speak? Alicia's silence is unsettling, both to the reader and to Theo. It provokes both his curiosity and his ego. Curiosity because he doesn't understand why, and ego because he believes he can "save her".
