It's more of a prequel to the first book, but you don't have to have read it to get this one. The two stories are very separate, taking place in different countries and with an almost entirely different cast of characters.
There is, indeed, a lot of romance. If anything, I'd say the romances (because there is some question about who the protagonist will end up with) are the main plot, while the political intrigue is secondary.

YES! ( for the romance!) I guess its probably considered a sequel but you don't have to read Graceling to read this book. there are a few references, that if you've read Graceling you would get (they are small) but otherwise you would probably just pass over them. ( like if you watch fuller house but have never seen the original full house)
