I think it was a literary device from the author. Anna couldn't save her husband and daughter. In fact, she feels as if she's to blame for their deaths. But, in the end, they saved her -- through the dream where she woke up and discovered Ethan to the description of Ed at the very end, which gave her enough time to break away from Ethan.

I too was curious if there was more to the back story. Sure he was adopted at 5 but that doesn't mean his bio dad couldn't also have been his adopted dad. The father's anger at Kate made me wonder if they had history.

I have the same question! And right at the beginning of the book when she was ‘talking� to Ed, she said they have a son, a teenager. Why did she say this???

i also think that anna was simply stalling.
even though ethan was "adopted", in the end, i think we are supposed to believe that alistair was his biological father. when he comes over looking for Katie, and sees the chess set, anna asks if he plays. he says something like, no but i used to. and the way katie talks about alistair in general. i didn't think much of it during the first read, but i'm flipping through again and i see it.

I think she was just stalling. Nothing else in the book suggested that Ed had had a son.

She was just stalling. I guess in the moment Ed's personality was what came to her, so she lied to Ethan and passed off Ed's personality as Ethan's father's.

Well, now, that's a great question. The author never tells us HOW it came about that Ethan was adopted. Now, I sort of thought maybe Alistar was Ethan's real father and Alistar had had an affair with a drug addict and then Jane was willing to adopt Ethan. But either way, I didn't believe for a second, never, ever, that Jane would have done what she did to Ethan's biological mother. NO WAY!!!
